Fairfield Greenwich - one of the largest institutional investors in Bernard Madoff's alleged $50bn Ponzi scheme - has been accused of "flagrant and recurring misrepresentations" that "give rise to fraud" by Massachusetts Secretary of the Commonwealth William Galvin.
